Monitor Visits: Do not leave your binders and paperwork in Monitor Rooms

Hello CCTO Clinical Staff,

There are been several occurrences of clinical staff leaving binders and/or carts full of study information and PHI/PII materials in monitor room FC163 at 800 Welch Road.

To be compliant with HIPAA requirements, do not leave these items in the monitors rooms FC163 and CC2108, as both locations are in public areas and documents could potentially be at risk to be misplaced and/or viewed by other monitors and non-CCTO persons, or recycled inappropriately.

Please make sure you pack up and clear ALL documents, loose-leaf paperwork and binders
from the monitor room AT THE END OF EVERY DAY.
